Requesting Medical Records
The Manette Clinic has partnered with MediCopy Services for the secure release of Protected Health Information (PHI), also known as your medical records. Your record may include medical history, diagnoses, treatment plans, lab results, and reports from past tests or procedures.
To request a copy of your records, a signed authorization form is required. You can complete the authorization form online here. If the patient is a minor or unable to sign, a legal guardian’s signature is necessary.
Once submitted, please allow two business days before contacting MediCopy to check the status of your request.
After Submitting Your Authorization:
- MediCopy will process your request within 15 business days of receiving your completed form.

For mailed records, allow additional time for delivery.

For emailed records, a secure, password-protected link will be sent to the email provided. Be sure to check your inbox and spam folder, as the link will expire for security purposes.
