Position Overview:
We are seeking a dedicated and compassionate medical provider to join our team, focusing on delivering high-quality care within the community. This position offers a smaller, community-focused patient panel, allowing you to provide personalized, patient-centered care while fostering strong relationships with patients. You will have a dynamic mix of responsibilities, including clinic coverage for acute patients and managing overflow from other providers. As part of our collaborative care team, you will play a key role in ensuring seamless, holistic care for both community-based and clinic patients, with a strong focus on continuity, prevention, and well-being.
Key Responsibilities:
- Provide comprehensive, evidence-based care to a smaller, community-focused patient panel, ensuring personalized attention and a proactive approach to health management.
- Deliver in-home or community-based care to patients with chronic conditions, mobility challenges, or other health needs, ensuring flexibility and accessibility in treatment options.
- Collaborate closely with clinic-based providers to manage acute care needs and overflow patients, ensuring seamless transitions between community care and clinic-based care.
- Actively participate in team-based care planning, contributing to an integrated, patient-centered healthcare approach that prioritizes continuity, prevention, and overall well-being.
- Maintain accurate and timely medical records in compliance with clinic protocols and regulatory standards, ensuring efficient patient care and confidentiality.
- Engage in quality improvement initiatives to continuously improve patient outcomes and optimize clinic operations, maintaining alignment with the clinic’s mission of accessible, compassionate care.
- Provide support and coordination for patients transitioning between community-based and clinic-based care, ensuring consistency and seamless healthcare delivery throughout their journey.

Qualifications:
- Valid licensure as a Nurse Practitioner, Physician Assistant, or Physician in the state of Washington.
- Board certification in Family Medicine, Internal Medicine, or a related specialty.
- A strong commitment to team-based care, with a focus on delivering personalized care to a diverse patient panel.
- Experience or strong interest in managing Medicare patients and a solid understanding of value-based care models.
- Proficiency with electronic health records (EHRs) and remote monitoring tools, with a focus on enhancing patient engagement and improving chronic care management.
- Excellent communication and interpersonal skills, with a patient-first mindset and a passion for building trusting, collaborative relationships with patients and colleagues.
